What is a hospital support plan & why do I need one?

This plan is created to assist medical staff to care and support the client.  This plan outlines all the client’s plans in summary.  It tells the hospital staff if the client has any allergies to food or medications, outlines their medical conditions and what medications they are currently taking.  The plan outlines the client’s communication style.  Whether the client is verbal or non -verbal.  Their level of understanding of receptive and expressive language, and how to best communicate with the client. It will outline whether a client likes a bath or prefers a shower and what level of assistance is required. 

This plan will advise staff on how a client communicates pain or discomfort and if the client has any challenging behaviours.  The plan will identify those behaviours and the known causes.  This plan may refer you to others listed above for more detailed information.

It is advised to prepare an emergency folder that has the hospital support plan in it, all contact details of person responsible, local GP and copies of any relevant support plans that may assist medical staff in supporting the client. (examples here would be:  Behaviour support plan, allergic reaction plan, epilepsy management plan, mealtime management plan etc.)

Having a emergency folder and or hospital support plan enables even a brand new carer to support the client in a hospital situation as the plan gives the carer a detailed description of all the client needs
